GLP-1 & body composition
A few months into a GLP-1?
Lighter doesn't always mean leaner.
Up to 40% of GLP-1 weight loss can be lean mass, not fat. Here's how to tell which kind you're losing, and act on it.
The scale's down again. So it's all working. Right?
But your arms look thinner. Not leaner. Not toned. Just soft.
The stairs feel longer than they used to.
You hit a number you've chased for years, and instead of pride, there's a flatness you can't quite explain.
You wanted to feel lighter and stronger. This feels lighter, but weaker.
You tell yourself you're overthinking it. The feeling doesn't go away.
The scale says you're winning. Your body says otherwise.
You did everything you were supposed to. And you can't voice any of your concerns out loud. Because who complains about losing weight?
If this sounds anything like you, keep reading.
This is much more common than you might think.
The line we had no answer for
We're Henry and Jackson, two brothers from Wyoming, and our mom felt exactly this, seven months in and 44 pounds down.
Everyone said she looked amazing. She didn't feel amazing.
She'd said something on the phone that neither of us had an answer for.
"I look smaller. But I don't look leaner."
She had spent that morning in a pair of jeans she had kept for fifteen years. They fit.
She still didn't feel like herself in them, and she couldn't work out why.
You already know a GLP-1 can cost you muscle. Everybody's warned you. But nobody ever gives you a way to know whether it's happening to you.
Our mom didn't have one either. She bought the protein shakes, the resistance bands, a $30 body-fat scale off Amazon.
But still no real plan to protect her muscle.
Here's why what she said on the phone that day matters.
In the landmark trial that launched the GLP-1 category, researchers measured not just how much weight people lost, but what kind.
Other studies put it closer to 1 in 4 pounds. They disagree on how much. They agree it is a lot.
In a review of nine trials, three quarters of them women, about 30% of the weight lost was lean mass.
Beavers et al., Obesity, 2025.
Two people can lose the same 20 pounds.
One of them...
Ends up lighter and leaner.
The other...
Ends up lighter but weaker, in a smaller body that needs fewer calories than what feels normal.
Same number on the scale. Completely different outcome.
Part of the reason we had no answer is that the studies cannot give one for a single person. What they can say is who tends to lose more. A small 2025 study of 40 adults found older adults, women, and people eating less protein lost more muscle on these medications. The population number is not your number. The only way to find out which side of it you are on is to measure your own.
Haines et al., ENDO 2025 · J Endocr Soc, 2025.
Right now, you're building one of those two bodies. Which one is you?
- If you've quietly wondered whether you're losing the wrong thing, keep reading.
- If you look thinner, but not the way you imagined, keep reading.
- If you're getting the protein in, hitting your step-count, and still can't tell whether any of it is working, keep reading.
Every scale on the market can show you a number. None of them hand you a plan to act on it.
Seven months of the scale going down
Our mom started on her GLP-1 at 56.
After twenty years of diets that never held, the weight finally came off. Fast.
Then one afternoon she tried to get up off the floor after playing with her grandkids. Her legs wouldn't do it.
She blamed the "dips in energy" everyone talks about on these medications. We blamed "age."
It was neither.
It took a physical therapist to measure what her scale never could.
She'd seen him for hip pain that spring, so her old strength numbers were still on file.
He ran the same leg test, then stopped and asked her to do it again.
Close to half the strength in her legs, gone.
And here's the scary part: she had been feeling it for months.
She just had no way to check whether the feeling meant anything.
What muscle actually does
Here is why that matters so much.
Muscle is what carries the groceries. It's what gets you out of bed in the morning, and off the floor without even thinking about it.
Lose enough of it and you don't just get smaller. You get softer, and weaker.
The mirror usually shows it long before any test does.
Researchers have compared the muscle lost over a full course of treatment to around 20 years of natural muscle aging. That was what our mom was feeling when her legs wouldn't lift her off the floor, and it was the part she could still change.
Mechanick et al., Obesity Reviews, 2025.
The good news is: this works just as powerfully in the other direction.
If you protect your muscle while the weight is coming off, it can completely change the outcome of your journey.
Her scale was never broken
The thing is, none of this was our mom's fault.
Her bathroom scale was never broken. It was doing its job: measuring her weight.
Fat, muscle, water and bone, all added together.
We started calling it The One-Number Problem.
What the scale told her
One number
- Fat
- Muscle
- Water
- Bone
The number went down every week. It never once said which of the four had moved.
When that number went down, she had no way of knowing which one was leaving.
You cannot change what you cannot see.
The instinct that runs out
Now, maybe you're already taking steps to prevent this. Extra protein. 10,000 steps. Maybe even weights.
It's the right instinct, and it's where almost everybody stops.
It is also what four major medical organizations recommended in 2025 for people on a GLP-1: more protein, resistance training, and tracking your body composition.
Protein, on a GLP-1
The instinct is right. It stops here.
Fewer than half of people on GLP-1s reach the protein target that supports muscle maintenance.

Not because they don't care.
Because generic protein advice was written for people who can eat, and for the days right after a shot, that isn't the situation you're in.
Here's the uncomfortable question: is any of it working for you?
We couldn't fix it for our mom. So we built it for the next person. For you.
It's called the Orvi Protect & Grow System.
It shows you everything a regular scale can't.
The Orvi Body Scan reads your whole body through 8 electrodes, measuring your arms, legs, and torso separately.
It's the same segmental configuration used in clinical devices.
You stand on it for ten seconds, and both numbers load on a simple app.
A home scale isn't a lab, and doesn't pretend to be.
What it gives you is the honest weekly trend, so you always know which way your body is truly headed.
One MRI study found muscle quality can improve even as it shrinks. The studies disagree and outcomes are individual, which is exactly why the honest answer is your own trend, read weekly, rather than anybody else's average.
SURPASS-3 MRI substudy, Lancet Diabetes & Endocrinology, 2025.
And if you've already tried one of these, we know how that probably went.
That $30 scale told our mom her body fat had jumped 4% overnight, then swung back the next morning.
It was in a drawer within a week, and she was right to put it there.
It measured her legs, and guessed the rest.
But seeing it isn't enough. Not without a plan.
The big-brand scale at the electronics store costs a lot more, and puts your own data behind a subscription paywall.
And none of their products arrive with training and nutrition plans to protect your muscle.
Every Orvi Body Scan arrives with the plans that change what those numbers say.
Gives your muscle a reason to stay, from your own home, with a progression rule that tells you exactly when to go harder, and strength tests every four weeks to confirm what you're seeing on the scan.
Runs the Four-Week Review. Your trend, your strength, your food, your symptoms and your rate of loss, side by side, then what to do next, all compiled into a one-page Clinician Summary you can hand across the desk.
And when your body is ready, the Grow and Power plans are built to help you build muscle back, not just protect it. No other system covers both phases.
Measure, fuel, protect. Then rebuild.
See the split. Change it. Then prove it changed.
Here's how the other version goes.
The number on the scale keeps dropping. The right things keep happening.
At some point the loss stops, and she never finds out what she was losing on the way down.
The question stays open. She has no idea whether she did this right. She can only go off how she feels.
Our mom lived that version for seven months.
Your story doesn't have to end the same way.
For less than a single month of most GLP-1s, you can stop guessing.
What you're really losing, how to hit your protein target, a training plan, and a review every four weeks that tells you whether it's working.
Try the whole system for 100 days. If it's not for you, email us and we'll organize a refund.
The plans are yours to keep either way.
Nobody offers terms like that on something they have to talk you into.
And if you've read this far, you already know a single number was never the whole story. That is exactly the instinct this was built for.

P.S. After our mom said "I look smaller, but I don't look leaner," we spent weeks with no answer for her. Then we found the research. Two people, the same 20 pounds, two completely different bodies. The number the scale shows is identical. The numbers it cannot show are everything.
P.P.S. The first morning takes about two minutes. Bare feet, before food, ten seconds on the scan, and both numbers are on your phone before the coffee is made. That's the whole ritual, and it never gets harder than that.
P.P.P.S. You can't get the last few months back. You can decide what every morning after this one looks like, and you still have most of this journey in front of you.
